Obesity is epidemic in the United States. According to data collected in the third National Health and Nutrition Examination Survey (NHANES III), completed in 1994, 10% to 15% of children and adolescents are obese, defined as a body mass index = 95th percentile for children or adolescents of the same age and gender. Furthermore, between 1980 and 1994, the prevalence of obesity doubled.1 Because 65% of obese 5- to 10-year-old children have at least one cardiovascular disease risk factor and 25% of 5- to 10-year-old children have 2 or more, childhood obesity must be considered as more than a cosmetic problem. The prevalence of obesity, the recent increases in the prevalence of obesity, and the co-morbidities associated with obesity emphasize the urgency with which effective treatment and prevention must be developed and implemented in primary care settings.
