In this issue of The Journal, Rovet and Ehrlich report the first prospective examination of cognitive function in a small group of diabetic children who were followed up for 7 years from the time of diagnosis. On the basis of their preliminary data, they conclude that certain cognitive skills decline somewhat over a 7-year period, and this is most pronounced in children who experienced a hypoglycemic seizure. Their work extends previous research, conducted over the past 15 years, demonstrating that children with diabetes—especially those diagnosed in the first 5 years of life or those who experienced severe hypoglycemia—have a greatly elevated risk of manifesting impairments on neurocognitive measures. Citing the well-established link between intensive therapy and severe hypoglycemia, Rovet and Ehrlich recommend that children with diabetes managed by an intensive therapy regimen need to be monitored closely to prevent seizures.
