PT. Hasil Raya Industries is a company manufactur in the plastic industry, their products are bottles and plastic lid, such as drink bottles, detergent bottles, cosmetic bottles and other bottles. PT. Hasil Raya Industries produced by using injection and molding machines. Bottles and plastic lid are made using raw materials and supporting materials from suppliers and then their made the production into finished goods. Supply chain performance measurement system in the company has no definite standard so that research was conducted to analyze, identify a performance measurement system, and measure the performance of supply chain management of production. Supply chain management to be effective and efficient, it is needed mapping of a whole activity to company supply chain. Mapping activity supply chain has been done by production that uses Supply Chain Operations Reference (SCOR) method and this mapping was done from level 1 to level 3. SCOR method brought into relief scope of company activity from upstream to downstream. SCOR method have five element such us plan, source, make, deliver, return and each element has different KPIs. Based on identification KPIs have 15 KPIs can measured
