Isi artikelThe prevalence of cardiovascular disease increased in the elderly. In the post menopausal women, the cardiovascular disease increased rapidly. This is in line with the hypothesis that said estrogen was cardio-protective. The use of hormone replacement therapy (HRT) weather estrogen alone or combination of estrogen and progestin originally suspected will decrease the number ischemic heart disease events. Using HRT within a short time in post menopausal women can effectively overcome the vasomotor symptoms, reduce vaginal dryness, and urethritis. However, long-term use of HRT would increase the incidence of breast cancer, stroke, heart disease and vein thromboembolism. In a meta-analysis of estrogen-use studies, the risk of vein thromboembolism is 1.3/ 10.000 women per year, with an additional 3.2 events at the first 12 months and 1.2 events after 12 months. The use of combination HRT also showed an increase in risk of vein thromboembolism than women not using HRT. Another meta-analysis concluded that estrogen transdermal has smaller risk of thrombosis compared to oral estrogen. Aspirin is a medicine that has long been known and has anti-thrombotic effect. It has been used as a prevention and therapy in stroke and myocardial infarction. In the Women's Health Study (WHS), 100 mg of aspirin in healthy women age >= 45 years for 10 years showed a decrease in the risk of stroke in the aspirin group, but does not cause a decrease in risk of myocardial infarction. This insight can open us to consider the use of aspirin as primary prevention of vein thromboembolism for post-menopausal women who use oral HRT.
