In this issue of the Journal, Frank and colleagues present the case history and autopsy results of a widely publicized death in the summer of 2007 of a young woman who was taking part in a Phase 1–2 gene-therapy study of rheumatoid arthritis.1 Although the patient's death was initially attributed by some to the intraarticular gene therapy, because of a striking temporal association, the adeno-associated virus (AAV) vector appears now to be exonerated. The death is most reasonably attributed to disseminated, recrudescent infection with Histoplasma capsulatum, an environmental fungus that is present primarily, but not exclusively, in the midwestern United States.
