There are many methods in Total Quality Management (TQM), which can be used in product and service quality improvement. Multy Attribute Failure Mode Analysis (MAFMA) is one of them used to eliminate or reduce the causes of failure in order to prevent the repeating failure. MAFMA is a method developed from Failure Mode and Effect Analysis ( FMEA) intregating severity, occurance, and detectailty attributes with expected cost. In FMEA, scoring determined by the highest risk priority number (RPN), meanwhile in MAFMA weighting for the four attributes carried out with Analytic Hierarchy Process (AHP). The attributes becomed the criteria level in hierarchy structure and the potential causes as alternative level. PT. X case study showed that weight of severity, occurance, detectability, and expected cost respectively are 0.3498, 0.0659, 0.1322, 0.4521. the weight of potential failure cause which is DDR knife losing grip is 0.3165. |
