Optic Neuropathy at Pulmonary Tuberculosis Patients Using Ethambutol in
Atma Jaya Hospital on December 2008

Ethambutol is one of the standard regiment of Anti-Tuberculosis Drugs (ADT) for
pulmonarytuberculosis recommended by Pedoman Nasional Penanggulangan
Tuberkulosis 2007. Several researches that has been conducted shown that
ethambutol has ocular side effect which is optic neuropathy in the form of loss of
visual acuity and color perception disturbance after some duration with certain dose.
The purpose of this study is to know the incidence of optic neuropathy in the form of
loss of visual acuity and color perception disturbance at Atma Jaya Hospital’s
tuberculosis patients using ethambutol on December 2008 along with the duration of
therapy and the doses of ethambutol that were being used.This study is a descriptive
one using a cross sectional method. The samples which were taken according to time
interval weretuberculosis patients in wards and policlinic of internal medicine of
Atma Jaya Hospital that was having ethambutol as therapy between 1 December
2008 and 31 December 2008.Visual acuities of those patients were assessed by
Snellen chart and colour perceptions were assessed using Ishihara card. Collected
data were processed descriptively and tabulated into table and diagram. From 31
respondents in this study, men is the majority (58,1%) while women only 41,9%,
which 81% of them are economically in productive years (15-50 years old). The
doses of ethambutol used by the respondents were largely similar as the
recommendation of 15-24 mg/BW/day with variation in duration, majority of usage
between 3-6 months. From those 31 samples, there were no loss of visual acuity,
while there was 1 sample got colour perception disturbance but this sample needs
further evaluation to differentiate the cause of the disturbance, either by ethambutol
therapy or hereditary cause. The conclusion from this sample is the doses and
duration of ethambutol therapy at pulmonary tuberculosis patients of Atma Jaya
Hospital on December 2008 have been good enough so there was no significant
incidence of optic neuropathy in the form of visual acuity disturbance and colour
perception disturbance.
