Twenty-five years ago, care for patients with symptomatic coronary artery disease centered on the selection and titration of antianginal medications. Percutaneous coronary intervention (PCI), which was limited at that time to balloon angioplasty, was an alternative. However, the risks associated with the procedure and the rate of restenosis largely relegated it to second-line therapy for patients who did not have a response to best medical therapy. Fast-forward to the present. Coronary stents have revolutionized PCI and have reduced the rate of complications and the need for repeat interventions.
