The ethical principles which guide the development of new drugs are sanctioned by the ancient saying Primum non nocere, which has always guided the practical applications of pharmacological disconveries. An important question arises, however regarding the real limit set by this statement, especially when confronted by the necessity to carry out high risk therapy. An example can be found in the treatment of neoplastic diseases with a terminal prognosis where a compromise must be found, and where adverse drug effects are accepted in the attempt to prolong the life of a patient in danger.
