Abstract
The purpose of this study was to evaluate the effectiveness of a comprehensive substance use prevention program on participants' substance use levels, psychological adjustment, and self-esteem. The program, which was implemented in an alternative high school for only high risk adolescents, offered an array of services including individual, group and vocational counseling, life skills classes, and job placement. It was thought that if these services were offered, students would use them, and this would improve their coping abilities as well as self-esteem. And this improvement in their psychological well-being in turn would reduce their substance use. Participants (

= 294) completed pre and post-test self-report measures that collected data regarding their substance use behaviors in the previous 30 days, their psychological adjustment, and their self-esteem. To begin with, the HMC program, although created with the best of intentions, was unsuccessful. In general, it did not improve participants' psychological well being nor did it reduce participants' substance use levels. Surprisingly, the program actually seemed to worsen the situation. Many of those who initially reported no substance use behaviors, by the conclusion of the program, reported they had begun using alcohol and marijuana. Female participants showed an improvement in their psychological adjustment and self-esteem, but they also reported a significant increase in marijuana use. Males reported a similar increase in alcohol consumption even though they reported using many of the program services. Caucasian students showed the least improvement in psychological adjustment and self-esteem, while African-American participants showed the most improvement. Alarmingly, pregnant females actually showed a slight increase in marijuana use by the end of the program. Finally, and probably the most important finding of the study, was that offering a comprehensive array of services to high risk students does not guarantee that they will use them. Nor does using program services guarantee change in either psychological well-being or substance use patterns. Study design limitations and recommendations for improving the evaluation component of the program are presented.
