In November, 2005, a 41-year-old man presented to his general practitioner with lethargy, polydipsia, polyuria, and weight loss. He was diagnosed with diabetes mellitus, and treated with metformin and gliclazide—but his glycaemic control remained poor. A diabetologist prescribed insulin, on which the patient's blood glucose concentration was 5–8 mmol/L. However, the diabetologist also noticed that the patient was cachectic, and had hepatosplenomegaly.
