Nursing is a pivotal aspect of the care of the chronically ill throughout the world. One published review of the literature (Temmink, Francke, Hutten, van der Zee, & Huijer, 2000) has focused on six Western, industrialized countries and the innovations related to nursing practice and care for the chronic patients that are being implemented in these countries to produce new forms of health care. These innovations were found to generally relate to substitution of care, which is the shift of care to the most appropriate health care provider in an attempt to lower cost while providing quality care. The six countries that were represented in the literature (Ireland, United Kingdom, Canada, The Netherlands, United States, and Sweden) were categorized according to their predominant health care systemusing the method established by Starfield (1994) and Westert (1997). The health care orientation was determined to be primary versus secondary by the availability of generalist primary services and specialist secondary services within the country. These categories were used then to determine whether the substitution of care innovations are different across the type of health care systemin place in the countries.
