We report 41 year old man with Dandy Walker variant presented with headache and vertigo. The patient never developed evidence of celebral involvement before. On neurology examination we found positive Romberg test but otherwise normal. On computed tomography (CT scan) of the head showed ventriculomegaly. While on Magnetic Resonance Imaging (MRI) showed enlargement in all ventricles system especially fourth ventricle due to cystic dilatation. The inferior vermian lobules were hypoplastic and the brain stem normal. This case is unique in that the patient has been entirely asymptomatic with this abnormality since birth. Dandy Walker variant usulally present as isolated cases of hydrocephalus in pediatric patients and related with brain embryology malformation.
