For those of you that have been home care nurses for years, you know that since the hospital DRG system was started, patients have been discharged to the home environment sooner and more acutely ill. Many of us wondered if soon we would be running an Intensive Care Unit in the home. A large percentage of the patients we care for in the home have a primary cardiac diagnosis, and of the remaining patients, many of them have a comorbidity of heart disease. We have all cared for patients in the home that had slow or rapid irregular pulses posthospitalization. They have a history of atrial fib or bursts of supraventricular tachycardia or other dysrhythmias, but without some type of cardiac monitoring in the home, we were unable to verify this. Technology in the home has become more advanced, and we have the opportunity to now monitor these types of patients in their homes using telemetry, which is similar to the monitoring systems used in hospitals.
