The Community Health Accreditation Program (CHAP) has undergone a recent reorganization, and in addition, the CHAP millennium standards were recently released. At this time, CHAP’s CORE standards of excellence and the Home Health and the Hospice Program standards of excellence are available. Additional standards are planned for release during the next few months.
