Pabrik II unit Pupuk Phosphate PT Petrokimia has never been put a risk assessment methods to assess hazards in their environment of plant. The research aim were to identified hazards and risk, also to give a rank of those hazards identified in order to analyse risk priority as an effort of safety control. This was an observational-analytical, the methods used was preliminary hazards analysis to identify all hazards at the plant, after that a risk assessment approach was conducted based on AZ/NZS 4360:1999. Data were collected by field observation, interview and secunder data owned by the company. After priority of risk was revealed, recommendation of a preventive and corrective action was given. The result showed that there were 5 major hazards sources in 16 detailed hazards, hazards which had a highest risk was an explotion of a nature gas. From the hazards ranked by the level of risk, its hoped that the company could increase hazards controling and monitoring as the implementation of safety program.
